They probably had plastic surgeons lining up to pay them to do the procedure.

They reportedly make six figures per social media post and the exposure for the surgeon would outweigh the cost

Six figures per post?

I'm not looking for a deep dive into social medianomics. While what you said may be true regarding surgeons lining up and the money those "ladies" make off the internet, the money made off of being a moron doesn't exonerate them from being morons. Steve-O is another example. Not that the Cavinders are at that level (yet), but saying "yeah but he makes/made a ton of money doing all that stupid schidt" does not make what he does less asinine. Here's a thought - how about doing something useful for society in any capacity to make a living? And yes, the irony and humor of an anonymous dilldough who goes by "BigDikDaddyFromCincinnati" calling out skánks for doing skánky things on the internet is not lost on me.

Potential on how this plays out

The easiest - They both marry a rich pro athlete
The intermediate - a few years left of still being the hot athletes but now with some nice C Cups doing some swimsuit stuff, influencing on social media then they marry a rich athlete
The potential outcome - 1 and 2 both happen and in a few years they just kind of fade away.
The way my messed up brain thinks it will go - the first three things listed happen but over time the looks and the fame start to go and then all of the sudden you have twin Tan Moms.

I guess I simply am struggling to wrap my pea brain around the concept of being paid six figs for a (stupid) TikTok video. Without divulging too much, I play a role in overseeing some social media that my company puts out. I contacted someone with about 800k followers in a similarly aligned business to ask about doing a collab. I thought his price of $5k plus expenses was outrageous and politely declined. If that dude is getting $5k for doing simple videos with a smile, more power to him. If the Cavinder skánks are getting 10-20x that, ¡marrón! More power to them I guess. At some point though, their looks will be a thing of the past and those fake tiddies gonna look like a pair of tennis balls hanging from tube socks.
