Made the right read, just completely missed the throw. Completely unacceptable or defensible

It's not even close. That's the problem. He missed so badly, you would have thought that he doesn't know that he's supposed to throw to the guy with the white jersey. There's no way an experienced QB misses that throw, that badly in that situation unless they are just bad.
