Unfortunately, the forms aren't online, so you'll have to send a self-addressed stamped envelope to CIS HQ to request them and then fill them out using a #2 pencil. PM @Andrew for the address.

Not a bad idea for ease of locating, but sometimes the surprise the booty gif is the greatest gift of all. Indeed, I’ll argue that 1-2 booty gifs should be embedded into every thread. Kind of like the Daily Double on Jeopardy. You know it exists but you don’t know where it is or where it might pop up.
